Cost: $50.00 per player
Last day to register for 2008/09 year is May 25, 2008 Any coach registering after this date must contact Jenell Bourassa at Jenell36@aol.com
NEW players must supply a copy of the player’s legal passport or certified birth certificate, a US Club player registration form and 1 inch by 1 inch photo.
RETURNING players must supply a 1 inch by 1 inch photo and registration fee.
COACHES if you are adding an assistant coach you must have them go to the US Club website www.usclubsoccer.org and fill out the background check information. Then you must submit a coach’s form for me to enter him into the system.
We accept payment in the form of cash, personal check, money orders, and cashier’s checks. Personal check must be pre-printed with a residential address. Payment is made out to Tracy Futbol Club.
Note: Uniforms are NOT part of the registration fees, nor are they included in the registration payment. Contact Joan Piazza at piazza6@sbcglobal.net to place your uniform order and then submit your payment to Treasurer Michelle Farmer, she can be reached at Lunchstop15@aol.com. You have the option to order warm ups at the time of your uniform order or anytime during the year.
